Most patients start at 50 mg as needed about one hour before activity. The tablet may be taken from 30 minutes to 4 hours before. Sitting blood pressure after 100 mg fell on average 8.3 / 5.3 mmHg in healthy men, most notable at 1-2 hours, back to placebo by 8 hours. That small drop is why a nitrate on top is a different story. The drop itself is not a reason to skip a stable antihypertensive.
Elderly volunteers showed about 84 percent higher sildenafil AUC and more than a doubling of the metabolite AUC versus younger men.
